This letter would be sent to a credit bureau if they ignored your previous requests. Remember to include a copy of your original mailing and the certified receipt. Also remember to only send this letter to the bureau(s) that are reporting the derogatory information. If you send this letter to one of the three bureaus that is NOT reporting the information, you take the risk of having the derogatory information added to that bureau report. Send proof of current address, proof of social security number, and send all correspondence via certified mail.

I have included a copy of my original mailing and my mail receipt for your records as well as mine. Please take immediate action to rectify these issues.
